Bob looking at the plate it appears that the axle was made by GKN so try an internet search for GKN Forklift axles, somebody somewhere must have the parts you need.

Jeremy, dana corp purchased GKN HEAVY in 1995. i spoke to dana rep. last year , he told me the part numbers were good in there system. he could not locate parts here. he told me my best bet was to inquire in the UK. i found the gear set at FULTON forklift in Ireland but the owner of our company muddled that up . so i,m of the option that Carol at FULTON may have had enough of the back and forth nonsense with the owner here. anyways ,still looking. thanks for your shoulder , bob

Bob this is a long shot but have you considered trying to get the bits made by a gear specialist or would that be too expensive?

Jeremy, dana /spicer are the OEM supplier of parts in the axle. they quoted 5 sets (minimum run) at 11,378.50 GBP plus EX works UK. they have been discontinued. anybody need the other 4 ? lol
