Crane rollover

This happened on a minesite where I was Colliery Engineer back in the 1980s. A 150 ton Grove crane was called in to do several lifts for us. The rig travels on 9 axles to comply with road weight limits and requires a semi trailer to accompany it with the items they remove for travelling etc. These are the counter weights, out riggers etc. After doing our work, I signed of the works order and left them to strip dwon and depart. For some reason the crane driver swung around and of course, it toppled over, missing all our men and machinery and demolished their own support truck and totally wrecking the crane, several millions of dollars worth. The Mine Manager, (my boss) wanted to prosocute me for allowing a dangerous act until I showed him the signed off docket!! Driver was sacked on the spot and the company removed the rig on a lo-loader at night under tarps to aviod publicity. As they say **it happens.
