probably missed a gear and brakes would not hold, once had these happen when I was following uncle Ben's low loader coming off Lee Moor he had an d8 and ripper with blade under driver missed gear and it rolled back and the winch hit front of 631 I put the boot down and he was soon up the hill. When we got just out of Plymouth we park up.The police escort came and see me and the driver and said you always now a local driver they select 1st at bottom of the Fxxxxxxx hill and that was all that was said about it. now there would be inquiry etc

What he did wrong was that he parked the trailer in an uphill slope. That puts less weight on the rear wheel on the trailer tractor. That means that the grip of the tires (and the parking brake) will be less than usual. And when the Cat goes backwards it reduces weight on the trailer tractor until the tractor starts to slide on the road.

I have done that. I had reversed in a very steep uphill slope and parked my MB-Trac. On the trailer was my digger. I started driving the digger to the rear end of the trailer and the the tractor, the trailer, the ramps and of course the digger all started sliding downhill uncontrolled. I immediately changed from being a comfortable driver to being a terrified passenger!
I almost sh*t myself but somehow I realized that I had to drive the digger forward to get more weight on the rear wheels of the tractor. It worked!
The skid marks on the asphalt from the rear tires where just 3 meters but it felt like I had slid several kilometers.

And then I used 4wd on the tractor to get all 4 wheels active on the parking brake and that made it.
